Transportation Forms

Please note that desired changes to a student's transportation services will not go into effect until the proper forms have been submitted and received by the department's Routing Specialist. Please submit all forms at least three to five (3-5) days in advance of the desired start date.

Transportation Forms

Transportation Change Request (Daycare Form)

Request form to change a student's transportation anytime during the school year. Please submit at least three to five (3-5) days in advance for processing.

If there are any changes from the current school year's transportation arrangements, submit this form before July 31st to start transportation for the beginning of the new school year. No further changes for the new school year can be made two weeks prior to school starting.

We only transport to daycare centers and private sitters within the district.

Form- Late Bus Authorization

This form is required to authorize students in third, fourth, or fifth grade to ride the late buses home on Tuesday, Wednesday, and/or Thursday afternoons.

Form- Change of Address

Required for students that have moved within the district. Two (2) proofs of residency are required when submitting this form.

Form- Request for Transportation to Non-Public Schools

Request form for transportation to a non-public school. Requests for the following school year are due annually by the first Monday in April. Two proofs of residency are required. Birth certificates are required for new students transported by the district. Please note that only non-public schools that are 15 miles from a student's primary residence (most direct route) are eligible for transportation services.

To request summer school program transportation, fill out this form and submit to the Transportation Department.
