Attention High School Middle
School Athletes:
Free sports
physicals for the 08-09 school year are being offered on Wednesday June 18
and Thursday June 19 from 11AM-3PM. Physicals are by appointment ONLY and
can be scheduled through your school Health Office.

All student
athletes MUST have a current physical on file in their Health Office in
order to participate in interscholastic sports.

Change in New York State
requirements
for physical
examinations

| The health office staff is eager to cooperate with you, as
parents, to be sure that your child is healthy and ready to
learn. Please keep your school nurse informed of changes in
your child's health, allergies, injuries, etc. |
| The nurse, or another staff member, is
responsible to provide first aid should an emergency occur at
school. It is necessary to inform the health office of telephone
numbers where parents may be reached
if an emergency arises. We also request names and phone numbers of 2
reliable friends or relatives who could assume temporary care of your
child if illness should occur and you cannot be reached. |

When deciding whether or not to send your child to school,
the following guidelines may be helpful. Please keep your child home
if: |

A fever (100 degrees or higher) is present or has been in the
past 24 hours.

The child has a cold with a heavy nasal discharge
and/or a persistent cough.

The child has a sore throat, vomits before school, has
an unidentified rash or is tired and generally not well
