BSCSD Student Athlete Survey
BSCSD 2008
Fall Sports Information
Any student who
desires to participate in an interscholastic sport at Ballston
Spa must first:
o Be
enrolled as a bona fide student in the BSCSD in grades 7-12.
o Have
record of a physical examination, conducted by the school or
your personal physician, and documented on the BSCSD
school/sports physical form. (In addition, the school
physician must approve the student for participation.)
o Complete
and submit all required documentation to the appropriate
school health officer (school nurse) by August 1, 2008 for
high school sports, and by August 18, 2008 for modified sports.
o Required
documentation forms (physical
form & green
card) may be obtained from the middle school or high school
main office during the summer. School office hours are 8:00am to
3:00pm. Please be advised that completed documentation submitted
after the aforementioned dates may delay your
child’s eligibility to participate in tryout/practice session(s).
